Infusion Pharmacy Services Coordinator - Infusion Services - Day

Hours: 8am- 6pm

Every 8th weekend, 9am-5pm

General Summary

In partnership with Team Pharmacists, provides outstanding customer service and support to the patient, pharmacy staff, and patient medical providers. Processes orders, ensures the patient has all supplies and equipment needed for service, and arranges/coordinates the delivery of home infusion medications and supplies to the patient's residence, ensuring all billing paperwork is present prior to delivery.

Essential Functions:

  • Organizes daily activities including patient work and phone call order.
  • Makes initial patient contact for new referrals. Explains WellSpan Infusion services, patient expectations and patient anticipated financial responsibility.
  • Coordinates delivery services and communicates delivery information effectively to patients, caregivers, nurses, pharmacists and distribution personnel.
  • Under Pharmacist supervision, processes new orders, creates delivery ticket, pulls required pump if appropriate and updates order expiration dates.
  • Enters Medical information into patient record in Pharmacy system including catheter information, equipment, shipping directions and documented phone conversations related to coordination of patient care.
  • Reviews financial documents required for the patient's delivery and follows up with Insurance Verification Specialist with any questions.
  • Assembles new patient packet with pharmacy information, patient rights and billing paperwork for patient signature.
  • Processes electronic billing of medications to required insurances.
  • Answers phone calls from patients for respective pharmacy team.
  • Makes Routine patient follow up calls, assessing the patient medication and supply needs. Refills orders as needed.
  • Produces pick up tickets at time of discharge from service to return equipment to WellSpan Infusion Service.
  • Works with Billing and collection team to resolve delivery ticket related problems.
  • Stages the prescription and products for the pharmacist to review and verify.
  • Hands off patient packet to pharmacist with prescription order.

  • Ensures patient confidentiality.
  • Maintains established policies and procedures, objectives, quality assessment and safety standards.
  • Works with team of other pharmacists, pharmacy technicians and other pharmacy to ensure quality of care to all patients.
  • Provides outstanding service to all customers; and practices fiscal responsibility through improvement and innovation.
  • Participates in department projects as assigned; makes proactive recommendations in updating department resources and reference materials.
  • Trains new staff and provides on-going support and instruction.
  • Actively supports process flow improvement and provides constructive feedback to Supervisor.
  • Resolves patient complaints.
  • Establishes and maintains files and records on an ongoing basis.

Qualifications

Minimum Education:

  • High School Diploma or GED Required

Work Experience:

  • 2 years Technician experience. Required and
  • 1 year Customer service related experience. Required
  • 2 years Technician experience in home infusion. Preferred

Licenses:

  • Certified Pharmacy Technician Upon Hire Required

Courses and Training:

  • Medical terminology. Upon Hire Required
