Heart and Vascular Program Administrator

Location: WellSpan Health, Lancaster, PA
Schedule: Full Time

General Summary

The program administrator is responsible for developing, implementing, and managing the comprehensive Heart Failure program and other assigned programs within the heart and vascular service line. This system level role works collaboratively with multidisciplinary teams and involves developing and implementing programs to improve and expand services and ensure that the department meets organizational goals. A key focus is on developing new programs aimed at improving cardiovascular care and patient outcomes. The role involves collaborating with clinical teams, managing performance metrics, and driving strategic initiatives to enhance service quality across the system.

Duties and Responsibilities

Essential Functions:

  • In collaboration with the Service Line leadership team, assumes responsibility for the development, implementation and management of the WellSpan Health Heart Failure Program.
  • Serves as the expert on all aspects of heart failure care to health care professionals, staff, patients, families and the community.
  • Develops and facilitates ongoing professional education programs for key stakeholders such as Emergency Department physicians, hospitalists, EMS providers, nursing, etc..
  • Develops and implements patient educational material for all patients who may benefit from a comprehensive heart failure services program.
  • Develops, coordinates and participates in community awareness programs about heart failure, diagnosis and treatment options.
  • Serves as Co-Chair of the appropriate Clinical Effectiveness Team(s) to facilitate standardization of best practices and collaboration among team members.
  • Organizes services in a manner to ensure timely and appropriate access and triage of services.
  • Facilitates the collection of performance measures to serve as a means to evaluate the overall customer satisfaction, financial performance and long term outcomes. Utilizes measures to identify and implement opportunities for improvement.
  • Ensures consistency and standardization of the program in each market across the System.
  • Coordinates and facilitates patient flow, communication and information flow between inpatient to outpatient care settings as needed.
  • Develops, maintains and updates all required documents for regulatory and/or accreditation bodies such as Joint Commission and Department of Health. Ensures that all mandated measures are collected and submitted in a timely basis and the hospitals are prepared for site visits.
  • Maintains awareness of market share to assess competitiveness of service. Develops and implements plans to optimize market share of Heart Failure services.
  • Works with Marketing staff to effectively educate community and referring physicians about the program.
  • Provides leadership for clinical and administrative process redesign.
  • Provides supervision and oversight to Heart Failure Program coordinators and or advanced practice clinicians at each System site.
  • Ensures that processes and programs are in place to support the success of the heart failure program with all participating hospitals

Common Expectations:

  • Is an active member of hospital and community committees that are related to heart failure patient care and community awareness. Represents the program at local, state and national meetings.
  • Serves as a liaison with pre-hospital providers, EMS, acute rehabilitation centers, other centers, clinics, government agencies and associations.
  • Assists with the development of the annual budget related to the heart failure program, both operating and capital expenses. Ensures quality care is given while maximizing financial reimbursement and developing opportunities for cost savings.
  • Demonstrates motivation for learning through independent reading, professional networking and attending relevant conferences and seminars.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation

Qualifications

Minimum Education:

  • Masters Degree Required

Work Experience:

  • 5 years Experience in hospital clinical management. Required

Licenses:

  • Certified Registered Nurse Practitioner Upon Hire Required or
  • Licensed Physician Assistant Upon Hire Required
  • Basic Life Support Upon Hire Required

Courses and Training:

  • Board certified by American Nurses Credentialing Center Commission on Certification. Upon Hire Required and
  • Completion of an accredited program for nurse practitioners or physician assistant. Upon Hire Required
