General Summary

Coordinates patient care/services in the assigned setting to encompass a thorough continuum of quality care to improve patient outcomes. Develops, implements, and manages programmatic change to ensure the program is current with evidence-based practice. Responsible for programmatic quality data.

Essential Functions:

  • Develops, implements, and evaluates clinical care pathways including but limited to physician orders, protocols, and algorithms for arrhythmia care in accordance with current guidelines and evidence-based practice standards in collaboration with program medical director and leadership.
  • Designs, establishes, and practices a service of structured care coordination to provide ongoing support to patients, family members, and caregivers through the continuum of care.
  • Collects, reviews, and analyzes programmatic data. Presents programmatic data to program providers, multidisciplinary care team stakeholders, and leadership. Leads identified quality improvement initiatives in collaboration with program medical director and leadership.
  • Develops patient education materials and process with emphasis on cardiac arrhythmia pathology, symptom recognition, risk factor modification, and treatment options. Provides individualized medication, testing, and prevention strategy education to patients, family members, and caregivers.
  • Ensures program patients undergoing an invasive procedure have all pre-procedure testing, pre/post procedure education, follow-up testing and appointments, and guideline medications are addressed.
  • Collaborates with the research team to identify, enroll, and support research studies and registries as it pertains to programmatic patient participation, including data collection as needed.
  • Complies patient health information, with investigation into relevant test results, medications, lab results, and medical/surgical history prior to consult with programmatic cardiologist to coordinate best care for patient.
  • Tracks programmatic demand versus operational capacity and service access in collaboration with leadership.
  • Collaborates with leadership, marketing, and medical director to build referral patterns, as well as educate referring providers and the community of programmatic services.
  • Participates in community events and community awareness programs with an emphasis on arrhythmia risk factors, symptoms recognition, screening, and treatment options.

  • Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  • Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

  • Bachelors Degree Required
  • Masters Degree Preferred

Work Experience:

  • 5 years Cardiovascular Acute Care Nursing Required or
  • 3 years Electrophysiology Acute Care Required

Licenses:

  • Licensed Registered Nurse Upon Hire Required

Knowledge, Skills, and Abilities:

  • Demonstrate competence in program development and project management
  • Ability to analyze and interpret data
  • Excellent communication skills - verbal, oral, and written
